Please call in advance to schedule an appointment for a personal consultation. It is not necessary to have a physician referral. Please be sure to bring your insurance card with you for any insurance covered procedures.
Occasionally it will be necessary for us to change your appointment because of emergencies. We do everything possible to minimize any inconvenience to you. If it is necessary for you to change your appointment, please call in advance so we can reschedule your visit.
