If you do not cancel your appointment at least 48 hours prior or if you are a no show, this payment is non-refundable. This amount will be refunded if you cancel your appointment no less than 48 hours prior. If you schedule surgery, this amount will be deducted from your surgery quote.
All insurance based consultations will be submitted to your insurance provider. Any copayments will be due at check-in.
